Here you can download and print various forms you will need for your visit to the Scoliosis Center. By filling these out ahead of time, you can save time during your actual visit.

How to get ready for your first appointment

  • Bring all related medical records and original diagnostic films (X-rays, MRI, etc.)
  • Bring a valid insurance card and driver's license.
  • Bring a list of your current medications.
  • Bring the referral from your primary care physician, if required.
  • The required paperwork can be downloaded and printed from the pdf file above.

Insurance Coverage

We accept many HMO and PPO insurance plans and Worker's Compensation. In fact, we accept more than 80 managed care plans. You should always check when making an appointment to ensure your insurance is accepted by the physician that you will be seeing.
